I hereby give my participant to attend the MYJO Jazz Camp

I agree to pick up my participant from The Sanctuary promptly at 4:00 pm daily.

I understand that my participant must provide their own instrument if they are a woodwind, brass, guitar or bass guitar player.

Consent for Photography and Videography: I, the undersigned parent/guardian of the participant, hereby grant the Montgomery Youth Jazz Orchestra (MYJO) and its representatives permission to photograph and video my child during the MYJO Summer Camp activities. I authorize MYJO to use such photographs and videos, which may depict my child's image or likeness, for promotional materials, educational activities, exhibitions, or for any other use for the benefit of the program. Release Details: I understand that the photos and videos may be used in print publications, online publications, presentations, websites, and social media. I also understand that no royalty, fee, or other compensation shall become payable to me by reason of such use. Permission is granted without time restrictions, for any and all purposes associated with the activities of MYJO.
